Transition Planning

If your son or daughter has a Statement of Special Educational Needs they will have a Transition Review in Year 9.

The Transition Review is a meeting to help your son or daughter plan their future.. You and your son or daughter will be invited to this meeting as well as your son or daughter’s Connexions Personal Adviser.

After the meeting the school will write a Transition Plan. This is a document that outlines your son or daughter’s plans for the future. It will detail:

  • what support your son or daughter requires in their transition from school to adult life
  • who will provide this support and where this will take place
  • what will happen during your son or daughter’s last years at school
  • any additional needs your son or daughter may have and how these may be met.

Tranisition Plans are reviewed each year your son or daughter remains at school.
